Service Description

New patients only. We accept some Aetna, Anthem, and Blue Cross/Shield PPO plans. We also accept Cigna and some United Healthcare Insurance plans. Please contact us to verify insurance. We require a minimum of 48 hours to validate all insurance plans, and your appointment will not be approved without this information.

Cancellation Policy

Cancellations / Rescheduling, must be done at 24 hours prior to your appointment to avoid the cancellation fee, and 48 hours before appointment for after hours appointments (those not on the website) and double appointments. Cancellation fee is $50 for a follow up appointment, $100 for new patients and double appointments. How to Cancel/Reschedule: 1.) Go to and go to the Book Online page. 2.) Click the "login" gear in the top right corner. 3.) Access your account by entering the email address used to make your appointment. You may need to create a password. 4.) Make the changes to your appointment. Our office is not open daily so you MUST cancel electronically. You may also send a text message to 213.394.6554. Voicemail messages will not be received, and are not considered a cancellation.
