Service Description
New patients only. We accept Aetna, Anthem, Blue Cross/Shield, Cigna and United Healthcare PPO Insurance plans. Please note, not all plans cover Dr. Levy's services. Please contact us to verify insurance. We require a minimum of 48 hours to validate all insurance plans, and your appointment will not be approved without this information.
Cancellation Policy
Cancellations / Rescheduling, must be done at www.drcoreelevy.com 48 hours prior to your appointment to avoid the cancellation fee. Cancellation fee is $100 for new patient appointments. How to Cancel/Reschedule: 1.) Go to www.drcoreelevy.com and go to the Book Online page. 2.) Click the "login" gear in the top right corner. 3.) Access your account by entering the email address used to make your appointment. You may need to create a password. 4.) Make the changes to your appointment. Our office is not open daily so you MUST cancel electronically. You may also send a text message to 213.394.6554. Voicemail messages will not be received, and are not considered a cancellation. Getting Here: We are located at 724 S. Spring Street in the Corporation Building, right next to the Food Hall. Use the Call Box (under Dr. Coree Levy) for entry. We are located on the 14th floor. Please ring the doorbell for entry as the door will be locked. If you are early, please be patient. Dr. Levy is either with a patient or cleaning the treatment room. Parking: Parking is located at 712 S. Spring Street. Look for the building with the big DTLA mural and the silver surfer on the roof. Let the attendant know you are coming to see Dr. Levy and he will give you a parking ticket. You will pay $5 in our office instead of the day rate. Do not pay the attendant. Questions?
